Just underneath the speedo on top of the triple tree is a gloss black round cover (single screw in the middle) that fastens to the main hex nut on top of the triple tree...

My cap was a little loose so I took it off to see what was underneath... there's a thin washer underneath the hex nut, but one of the sides was bent up... I'm guessing it was deliberately bent up to one of the sides to prevent the hexnut from loosening?

Is this correct?

Or did some joker bend it up just to get the gloss black end cap to go down?

Its bent to lock it. Its called a french lock. Has an internal lock on the stem then the bent uptab locks the nut. The internal lock could be a flat so it looks like a 3/4 moon, could be a tab that is either on the stem or the tree. They are one time use but many use them over.
